Electrical Flashover Incident

 

Image1On 03/04/2009 at 08:40 hours an electrical flashover was reported while the contractor was trying to find the information regarding 480V breakers. This resulted in injuring three contractor’s employees; one with first degree burns and two with 2nd degree burns. All the victims were transported to clinic for first aid treatment and then transported to Care Hospital for further examination and treatment.

These images are a good example of why Lock Out Tag Out is a must before beginning any electrical work.

Having the work leader verify the correct isolation at the energy source, ensure the tag is properly hung and all workers are aware of the hazard is very important. Make sure all workers are TRAINED on LOTO, always verify Tag Outs at the beginning of each shift and sign on and off of the Individual Accountability Form. Make sure proper PPE is available, if not- do not work!
