August 22nd           Industrial               Belleville                Owner of commercial strip mall received a shock while trying

                                                                                                to repair an electrical sign.  Sign was not locked out, person

                                                                                                was on an aluminum ladder while doing work.  Sign secondary

                                                                                               voltage was 680 volts to ground, when person received the

                                                                                                shock, he fell to the ground.  Person suffered broken back and

                                                                                                severed brain stem at top of spinal cord.

 

 

Critical Injuries:

 

July 10th                  Industrial               Markham               Worker doing testing of transformer, testing station used

                                                                                                foot pedal as safety interlock. There was also a bypass for this

                                                                                                interlock.  Secondary leads to transformer energized at 347

                                                                                                volts, primary side of transformer was at 16 kV.  The worker

                                                                                                made contact with the high side of the transformer, received

                                                                                                severe burns and was unconscious after the incident.

 

August 1st              Industrial               Stratford                Worker handling 600 volt welding cord, was unplugging

                                                                                                a welding machine from an extension cord.  As connection

                                                                                                came apart, worker dropped to the ground, convulsing.

                                                                                                Investigation showed that ground wire was not properly

                                                                                                connected where a second extension cord plugged into the

                                                                                                welding receptacle.  There was a second ground path

                                                                                                present through the welding machine into building steel.

 

August 3rd             Industrial               Vaughan                Worker on ladder moving armored cable.  Armor of cable

                                                                                                contacted a live wire, shock caused worker to fall from

                                                                                                ladder.  Worker lost consciousness from fall, taken to hospital.

 

August 20th           Industrial               Fort Frances          Three workers received burns from an electrical arc.  First

                                                                                                worker was a certified electrician, received 3rd degree burns to

                                                                                                both hands and 1st degree burns to face.  Second worker was

                                                                                                also a certified electrician, received 3rd degree burns to left

                                                                                                arm and 1st degree burns to face.  Third worker was a pre-

                                                                                                apprentice, received 2nd degree burns to left arm.

                                                                                                First worker was involved in installing new wiring for a power

                                                                                                box, breaker feeding box was turned off and locked out. 

                                                                                                Breaker was fed from buss bars located just above the breaker,

                                                                                                buss bars were exposed and energized at 460 volts.  First

                                                                                                worker had finished wiring in power box, was running new

                                                                                                wires to breaker, noticed that another wire needed to be

                                                                                                removed.  First worker got out uninsulated nut driver,

                                                                                                removed bolt making the connection, lost control of wire

                                                                                                as he began to move it.  Arc happened.  Second electrician

                                                                                                and pre-apprentice were close to first electrician, watching

                                                                                                the work when the arc happened.

 

September 10th      Construction         Otonabee               Worker installing cable at residence.  In back yard, was

                                                                Township              using a conductive pole to try to find a high-speed internet

                                                                                                signal.  Instead, made contact with overhead 4.8 kV line.

                                                                                                Worker knocked unconscious, both boots had burn marks.

                                                                                                Worker recovered, drove himself to local hospital.

 

September 11th      Industrial               London                  Worker on ladder, received a shock and fell off ladder.

                                                                                                Worker was elevator mechanic, doing work on elevator

                                                                                                electrics in pit area.  Worker received broken bones, loss

                                                                                                of consciousness.

 

September 14th      Industrial               Etobicoke              Electrician doing troubleshooting, used channel-lock pliers

                                                                                                to pull fuses from 600 volt disconnect switch.  Arc happened

                                                                                                as he was pulling the third fuse, received burns to face, hands

                                                                                                and body.  Worker thought power was off at switch, made a

                                                                                                mistake.  Switch checked over after the incident, no

                                                                                                deficiencies noted.

 

September 23rd      Construction         London                  Worker was tying in the controls to the transfer switch,

                                                                                                switch circuits were live at the time.  Arc happened,

                                                                                                worker has burns to wrist and hands.

 

September 30th      Construction         Red Lake                Worker in 44 kV substation, using non-insulted elevating

                                                                                                work platform.  Made contact with 44 kV, received 3rd

                                                                                                degree burns to about 45% of his body.

 

Non-Critical Injuries:

 

July 3rd                   Industrial               Waterdown           Worker received electrical shock from the flexible cord

                                                                                                from a machine. Worker experienced numbness in arm.

 

July 12th                  Industrial               Toronto                 Worker reached into 120 volt panel, touched a live wire and

July 17th                  Industrial               London                  Worker trying to move an electrically powered saw. 

                                                                                                Received a shock when one hand was on saw and the other on

                                                                                                a nearby metal gate.  Voltage of saw not given.

 

July 23rd                 Construction         Tiverton                 Worker was supervising welding work, part of his body was

                                                                                                touching the steel being welded upon while his hand was on

                                                                                                a nearby I-Beam.  Worker felt a tingle through his body.

 

                                                                                                Clause 6.3.2.4 of the CSA Standard W117.2-06, Welding

                                                                                                Safety Code, requires the WORK (ground) lead to be located

                                                                                                as close as practicable to the location being welded upon, to

                                                                                                ensure that the welding current returns directly to the source

                                                                                                through the work lead.

 

July 31st                  Industrial               Caledon                 Worker doing electrical maintenance, was restoring power by

                                                                                                turning on the main switch.  Worker wearing PPE such as

                                                                                                gloves, coveralls and face shield, also used the ‘left-hand’ rule

                                                                                                while closing switch.  Switch exploded, worker thrown about

                                                                                                10 feet.  Only injuries, worker did complain about sore hips.

 

July 31st                  Industrial               Sault Ste. Marie    Worker doing routine maintenance, lifted cover off battery.

  • Q:Can somebody kindly explain what do they mean by" using foot pedal as safety interlock while testing a transformer at a substation" ( ref. extract: accident occurance description in Mike Holts newsletter)

    Some testing machines (such as a doble power factor tester) have a safety interlock such as a handheld button, or in this case a foot pedal. In order for the test unit to operate the interlock must be depressed. If it is released for any reason the unit will shutdown. In this case the foot pedal had to be depressed by the technician in order for the test to continue.

    Now this is supposed to keep the technician safe by insuring that he cannot approach any 'live' parts while depressing the foot pedal. However as you can see if the the foot pedal is located close enough to the exposed terminals of the transformer for him to touch them while still depressing the pedal it defeats its original purpose!

  • I have encountered a lot of switchboard plugs and fusible switches that can be opened with the power on because the interlock is broken.

    Electrical equipment contains SURPRISES such as not as deenergized as advertised. I have also been told about an ITE vacubreak switch that failed to disconnect one of the 3 phases.

    And underground lines are not necessarily buried as deeply as they are supposed to be. Soil erodes, soil is regraded, prints are ALWAYS WRONG to some extent or another. First Energy requires hand digging when within 2 feet of a line locate. A lot of these problems occur when somebody is trying to do Trick and Precision Power Digging or is hand digging with a brute force digging bar.

    Expect surprises when you dig. Expect the prints to have something WRONG with them.

    When we were wiring that Aldi store in Streetsboro, Ohio the general contractor did not give us the parking lot print. The 7,200Y12,470 volt primary line ended up right at a parking lot curb and general contractor dug into the cable whilst digging trench for curb. Aldi could have let us consolidate 4 parking space into 2 extra wide spaces. I am also absolutely sure that the primary line ended with inadequate ground cover. Likewise, when the parking lot was regraded the bulldozer dug up our lighting conduits. Grading the parking lot could have been and should have been done right after the building foundation was bulldozed.

    On another job ( golf course for City of Akron ) the outdoor liighting contractor did not even ask us for a locate of underground lines, then bored out foundation right square on top or our duct bank.

    Besides the cost of digging deeply, if you bury an electrical line too deeply you have to derate it and deep enough requires artificial cooling, See NEC B.310.15(B)(4). I will even bet that in Hawaii underground lines at normal burial depth have to be derated from 20 degrees C ambient to 40 C and further derated for the poor thermal conductivity of volcanic soil.

    A farmer I knew asked Michigan DOT and General Telephone if he could cultivate the unused right of way along MI 66. GTE said," Sure, out lines are 36 inches deep." He set the plow for 22 inches, dug up an intercity line the had a segment only 18 inches deep because somebody got lazy. Farmer did not have to pay for the damage.

    I have also read about a rubbish worker who was working before sunrise, mistook a padmount transformer for a dumper, created loud boom. I have seen quite a few instances of a dumpster right next to a padmount transformer DISASTER WAITING TO HAPPEN.

    Gary Larson one time wrote a Far Side book titled, "Trouble is Brewing." Front cover had a dingo ( Australian wolf ) farm right next to a day care center. Dingos licking their chops during recess. He would have been funnier if he had also been an electrical engineer.

    When we lived in Sayre, PA GTE was undergrounding the telephone lines. Every natural gas service was 24 deep except ours which was only 12 inches deep. GTE cut our gas service with the cable plow. With a little more bad luck they could have started a natural gas fire.

    I have also seen 2 instances of water mains that only had an inch or 2 of dirt over it. Same doggone problem, different substance. One time back in high school, Equitable Gas was using a pnuematic torpedo to run a corrosion protection electrical cable just underneath the pavement. When the torpedo came out the other side of the pavement it poked a small hole in a West View Water Authority line that NOT underneath the road and NOT 3 or 4 feet deep. Water main actually had a small surface with no soil over it. Not as spectacular as blowing up a primary voltage. Another water main along Cedar Road in University Heights, Ohio is right in the tree lawn with exposed joints. Must be a lot of fun when Clevleand Electric Illuminating Company wants to install a streetlight pole - there is not even enough room for a homeowner to install a 6 inch deep landscape lighting cable. I will bet that the street signs are not pounded in as deeply as they are supposed to be. How the heck these lines do not freeze during a winter night is beyond me.

    Dig up the power line, break the water main, dig up the telephone, cut the gas - it is all the same set of problems.

  • A comment on the fact that the reports indicate most accidents occured in Canada-- but-- the fact is they do an excellent job of reporting to the public. We (USA) do a lousy job reporting bad things to the public, maybe for legal reasons or maybe because our government agencies just don't think we ( the public) need to know? But it is important to note this in your report.

  • Over at www.eng-tips.com is a posting by someone who survived digging into an unmarked 11,000-volt underground cable with a digging bar. The cable location was never marked because the utiltity FORGOT that this cable even existed.

    I have also read about a Boy Scout camp that was made to pay through the nose for digging up an intercity telephone line. There were no right-of-way markers because AT&T had somehow forgotten that this cable existed - they were somehow using it but were not maintaining the right-of-way. AT&T's underground cables are the best permanently marked in the business. So, if AT&T can develop amnesia, so can anybody else.

    There are also a kajillion abandoned oil and gas lines in Pennsylvania and a few other places as well. Actually, some of them probably might even still have gas or oil in them because no one was making the oil and gas people purge out the lines before abandoning them.

  • Just keep it up guys. The updates about the unsafety practices of my fellow Engineers are really making to be more careful in my activities and installations. i turly love everything i'v been getting.But can you guys help me in getting yhe latest building regulation types: Part A,B,C,E,L,M,P. And any u cld reach easily. I ld be grateful.
