2006 Fatalgrams and Fatal Investigation Reports
Metal and Nonmetal Mines

imageThe Mine Safety and Health Administration posts accident reports on all mine fatalities called “Fatalgrams”. Click here to view the Fatalgrams for 2006. To date there have been 18 fatal accidents. The detailed summaries which you can access by clicking the report button to the right of each incident provide lessons that can be learned by all of us. There have been three electrocutions so far in 2006 compared to the two that occurred in all of 2005.

This information was sent to us by one of our code forum moderators Tom Baker. Recently a good friend of his, a 75 year old electrician with 50 years experience was fatally injured at a gravel mine while installing a 480V branch circuit. Click here to view the Fatalgram for Tom’s friend.

It states "The victim contacted energized conductors while installing an electrical circuit. The building subsequently caught fire and burned before the primary circuit could be deenergized" A detailed report is being done and will be posted when it is finished.
