#1. Utility facilities are excepted from the NEC, they must follow: 1. Their utility wiring stanards and/or 2. The NESC. What is important is the service conductors brought under the NEC by the inside wiring contractor.
under NEC Article 250.24(C)all AC systems at 1000v or less must have a grounded conductor at the point of service with the electric utility regardless whether the inside wiring load is strictly three pole three phase or 2 poles single phase. There are some exceptions listed.
What I have always question but never had a "clean' answer. Why this article has a restriction to 100v or less. Many utilities service voltage to customers above the 1000v limit and the need to have a low impedance pass from the service disconnect to the utility power source remains. Hence a fault to service disconnect to case requires this low impedance path which the grounding rod will not suffice. Any help on this portion.....?
